Dr. Jake Yager is a Fort Wayne native and graduate of Carroll High School. He attended Indiana University – Bloomington for his undergraduate degree and received his Doctor of Dental Surgery degree from the Indiana University School of Dentistry in Indianapolis.
Dr. Yager prides himself on taking a patient-first and detail-oriented approach when caring for patients. He has a special interest in staying current with new dental techniques and technologies, allowing him to provide effective care to patients on a daily basis.
In his free time, Dr. Yager enjoys road bicycling, playing basketball and tennis, and hiking. He also values spending time with his wife Rebecca, his mini goldendoodle, Zoe, and his family and friends.

Dr. John Herber
Dentist
Dr. John Herber attended Indiana University (Bloomington Campus) for his undergraduate coursework where he majored in biochemistry. After his undergraduate study, he attended Indiana University School of Dentistry (Indianapolis) where he graduated with his Doctorate of Dental Surgery in 1992. During his undergraduate and graduate coursework Dr. Herber made the Dean’s List and he graduated with honors/awards from Dental School. After Dental School, Dr. Herber enhanced his education in dentistry by completing a one year graduate practice residency in Advanced Education in General Dentistry. After his residency education, Dr. Herber served for four years in the U.S. Air Force where he received the prestigious commendation medal for Meritorious Service. Dr. Herber was then invited to be an associate dentist in two separate practices in the Fort Wayne area after his honorable discharge from the Air Force. In August of 1996, eighteen years ago, Dr. Herber opened his own practice, “Herber Family Dentistry, P.C.” with just an assistant, hygienist & Elizabeth, his wife of nearly 25 years. The practice has since been blessed to its current team of 12 highly educated and well-trained staff members with Dr. Matthew Gotsch being the latest addition in 2013.
Dr. Herber and his wife Elizabeth have been blessed with two children, Lauren (21) and Zach (19). Dr. Herber has attended Wallen Baptist Church the last 17 years, where he has been a Senior High Leader in Lights Out Youth Group specializing in Apologetics and church history. Dr. Herber currently serves on the Fort Wayne Rescue Mission Board and is a volunteer at Matthew 25 Health and Dental Clinic. Dr. Herber is passionate about Apologetics (if you don’t know what this is, please ask him!) and reading. He also enjoys playing golf, traveling, music, and the comedy of Jim Gaffigan.

Born and raised in Fort Wayne, Indiana, Kim brings a sense of fun and community here at Herber Family Dentistry. Beginning her dental career back in 2002, Kim is a graduate of Anthis Career Center’s Department of Dental Assisting. Kim began working with Dr. Herber in 2002 as a part-time dental assistant and part time business assistant. Kim recently graduated from IPFW with a degree in Psychology. Kim is an avid sports fan. Angie was born in Huntington, Indiana where she now resides with her husband and two children, Hayley and Dalton. Angie graduated from Huntington North High School and immediately pursued a degree in Dental Assisting from IPFW. She graduated from IPFW in 1994 as a Certified Dental Assistant. In 2001, Angie went back to school and received a higher education in Expanded Duties Dental Assisting. Angie began working for Dr. Herber in 1999 when it was a “one man show.” She is thrilled at how the practice has and continues to grow. Angie enjoys spending time camping with her family where she can be found in a quiet area reading a favorite book. Angie has attended many continuing education courses that have and continue to prepare her for any situation that may arise at work. She loves her job and is looking forward to meeting and treating you as a patient.

Amber grew up in Syracuse, Indiana where she graduated from Wawasee High School in 2007 and immediately pursued a career in dental hygiene at the Indiana University-Purdue University of Fort Wayne campus. Amber graduated with an Associate of Dental Hygiene degree in 2011 in the top three of her class. She also earned the Hu-Friedy Golden Scaler Award from her professors which has given Amber a great start into the hygiene field. She now lives in Fort Wayne with her husband and dog, Wendy. She enjoys spending time with her husband, family and friends, playing volleyball and softball, and most of all playing with Wendy. Amber continues to attend continuing education courses to increase her knowledge and better her skills. She loves being part of the Herber Family Dentistry team and is very eager and excited to meet you and take care of all your dental needs.

Jamie grew up in Southern Indiana. She graduated from the University of Southern Indiana's hygiene program in 1993. After completing her B.S. in Dental Hygiene Education in 1994, she moved to Fort Wayne and has worked at Herber Family Dentistry since 1999. Jamie has enjoyed seeing Herber Family Dentistry grow into the practice it is today. What she loves most about her job is getting to know the patients and continuing to care for them over the years. When she's not working, she is busy with her husband and three children. She enjoys camping with her family and watching her kids play sports. You can feel confident knowing that Jamie has the experience and skills needed to treat you as a patient.

What she loves most about her job is being able to work with amazing people in such a great family friendly atmosphere at Herber Family Dentistry. When Carmela is not working she is busy trying to keep up with her young son, Dominic, alongside her wonderful husband Jason. She also enjoys playing volleyball, hosting bon fires in her back yard, catching up on her favorite shows, spending time with family and friends, and playing with her lovable boxer named Bella.

She then attended Ball State University for two years before transferring to IPFW and graduating with a degree in Hospitality Management in 2014. In her free time, she enjoys spending time with her family & friends, going to the lake, shopping, and vacationing in Siesta Key, FL . Ashley has been with Herber Family Dentistry since June 2016. She loves interacting with and getting to know each patient.
